Acne and Oxofulleram
Acne can be either non-inflammatory (whiteheads and blackheads) or inflammatory (red lesions which vary in size and which can be pustulent). Acne is associated with the Propionibacterium Acnes (P. Acnes) bacterium, which lives in the skin and produces waste products which can break down skin cells and thus activate the immune system, leading to infection.
Most over the counter medications for acne contain benzoyl peroxide (most common), salicylic acid, hydroxy acids, or similar ingredients that act as oxidizing agents. These oxidizing agents are applied to kill the P. Acnes baceterium and also to remove the top layers of skin and reduce clogging of the pores by P. Acnes waste products. As you know if you've used any of these common acne products, they can cause irritation, make redness worse, and cause dryness. Also, many people don't find improvement for their acne with these ingredients.
Oxofulleram, the patent-pending antioxidant ingredient available only in Combray, works in a different way to the above ingredients. Oxofulleram acts to reduce free radical concentrations which are known to be elevated in the skin of acne sufferers (Arican, O., et al., Oxidative Stress Levels in Patients with Acne Vulgaris, Mediators of Inflammation • 2005:6 (2005) 380–384). Exactly how this oxidative stress is implicated in acne is not clear, whether it is a part of the cause or a symptom. It is known that the immune system creates oxidative stress in the process of inflammation through the creation of free radicals such as nitric oxide. Reducing the inflammation associated with acne leads to less redness, smaller lesion size, and improved healing.
Effect of Oxofulleram on Markers of Inflammation
Description of Study and Results 0.3%
(wt.) formulation of Oxofulleram in grape seed oil was applied to in vitro skin models (Epiderm) to which UV was applied and a comparison was made with hydrocortisone, a well known anti-inflammatory (though too harsh to use on your face). Prostaglandin 2 (PGE2) is a well-known marker of inflammation, and levels of PGE2 are elevated in acne sufferers. It is one of the signalling molecules cells give off when under stress, and which lead the immune system to begin the process of inflammation (redness).

It can be seen in Figure 1 that Combray (the first bar) reduced PGE2 significantly. These results are consistent with oxidative stress reduction by Oxofulleram.
Solenne Survey Results
The strongest basis for understanding the effectiveness of Combray for acne are the results of Solenne's ongoing survey of Combray samplers. The sample is about a 12-day supply. Users who wish to try Combray fill out a brief survey describing what condition they have (acne, rosacea, eczema,..., which are all inflammation-related conditions) and how they would characterize their skin (sensitive,...). The number of respondents that then buy Combray after trying it for their condition is an accurate indicator of whether the respondent has experienced relief.
Over 1500 respondents have participated in the survey so far. Of the respondents listing acne as a concern, the conversion to purchaser of a full-size bottle ($156, 100-day supply) is the highest of any condition listed (rosacea, psoriasis and eczema also have a high rate of conversion), and about 4 times higher than respondents who list only anti-aging as a concern. The conversion rate is also high compared to industry standard rates. Among samplers who purchase the 3-pack sample (18-day supply) for $36, the number who end up purchasing is about 2.5 times higher, indicating that price is a factor (suggesting that a significant number experience relief but don't purchase due to price). This also indicates that 12 - 18 days is sufficient to see a noticeable result. Also, very few Combray users complain of irritation.
